Dosage Information

ED – General Adult Population

• Recommended dose: 1 tablet of Valif 20 mg (20 mg of Vardenafil) taken orally about 60 minutes before sexual activity. 
• Maximum limit: 20 mg of Vardenafil per day (1 tablet of Valif 20 mg).
• Important clinical information: Not more than one dose in 24 hours. Initial therapy may begin with 10 mg of Vardenafil depending on the patient’s profile. The dose may be adjusted between 5–20 mg based on efficacy and tolerability.

ED – Elderly Patients (over 65 years)

• Recommended dose:  5 mg of Vardenafil taken orally before sexual activity 
• Maximum limit:
  20 mg of Vardenafil per day (1 tablet of Valif 20 mg).
• Important clinical information: Initiate at 5 mg due to higher plasma concentrations in elderly; titrate cautiously. The dosage may be increased to 1 tablet of Valif 20 mg (20 mg of Vardenafil) if tolerated and clinically required.

ED – Patients on Moderate CYP3A4 Inhibitors (e.g., erythromycin)

• Recommended dose: 5 mg of Vardenafil taken orally before sexual activity. 
• Maximum limit:5 mg of Vardenafil per day when used with moderate CYP3A4 inhibitors.
• Important clinical information: Strong CYP3A4 inhibitors are contraindicated with Varif 20 mg. In combination with moderate inhibitors, maximum recommended dose is 5 mg within 24 hours. Do not exceed 1 tablet of Valif 20 mg (20 mg of Vardenafil), though lower dosing is preferred.

How Valif 20mg Works

The active component of Valif 20 mg is Vardenafil, a PDE-5 inhibitor medication that targets the NO-cGMP pathway.This pathway is involved in blood circulation in several tissues and organs.

Normally, an external trigger (such as sexual stimulation or nervous system signaling) activates the NO-cGMP pathway. Nitric oxide (NO) is released by the nerve endings or endothelial tissues in response to these stimuli, and it quickly permeates the surrounding tissues. It causes guanylate cyclase, an enzyme that transforms guanosine triphosphate (GTP) into guanosine monophosphate (cGMP) molecules, to become active.

As secondary chemical messengers, the cGMP molecules control other physiological reactions in the region. The cGMP molecules support the entry of potassium molecules into the cells and lower intracellular calcium levels. This promotes vasodilation, also known as the widening of blood vessels. As a result, the blood flow in these blood vessels is increased. Additionally, it also promotes smooth muscle tissue relaxation.

These reactions are necessary for the physiological responses to external stimuli to operate normally. These reactions are essential for the development of an erection, the operation of veins and arteries, and other metabolic processes.

After a sufficient concentration of cGMPs is achieved, an enzyme known as phosphodiesterase type-5 (PDE-5) is activated. This enzyme is constitutively present in the tissues where the NO-cGMP pathway is generally functional. It is released via a feedback mechanism when cGMP concentrations are high.

Any physiological effects caused by the cGMP molecules are reversed when the PDE-5 breaks them down. This results in smooth muscle toning and blood vessel constriction. To return the body to its typical resting state, these responses are necessary. The benefits, like vasodilation and relaxed tissues, would last forever if the PDE-5 enzyme were absent. These may lead to abnormalities like a protracted erection or severe pulmonary artery dilatation.

The effects of PDE-5, however, may be excessive or premature in certain people. Because of this, the cGMP molecules are unable to perform their roles for a sufficient measure. This anomaly in the vascular and the biochemical pathway results in conditions such as pulmonary arterial hypertension and erectile dysfunction.
